so you may have an enjoyable dental visit. It is important
to review the office policies in order to prevent any miscommunication.
Over the past two years the privacy act has made it very
difficult to contact your insurance and verify your coverage
or retrieve any information. As a result the following applies
if you have dental coverage.
As a benefit to our patients we can submit and receive the primary claims on your behalf. Although, if primary claims are outstanding past 60 days it is the responsibility of the patient to ensure the outstanding treatment is paid. We will do our best to assist you in the reimbursement of your outstanding dental claims.
Patients with secondary insurance coverage are requested to pay the difference at the time of the appointment and the insurance payments will be reimbursed back to you.
Statements will be sent when they are 21 days past due. If they cannot be
resent electronically, a manual claim will be highlighted and attached for you
to sign. Asterisks (*) are claims we have copies on file and have been sent on
your behalf.
We are aware that your time and our time are valuable. Please take
the time to note your appointment time. Your appointment is exclusively reserved
for you and the dentist or hygienist. If you require to change your appointment
we require two full business days to change or reschedule any visit.
Please note,
due to the high demand in Saturday appointments we need you to contact Laura
or Christine one week ahead in advance to reschedule or cancel the appointment.
This gives us time to call clients so they may have the time to arrange their
weekend, need be. It is very important to note if the appointment is cancelled
less than a week, we will not be able to reschedule another Saturday unless a
payment of $75.00 is made for each person from the previous Saturday missed appointment.
We apologize for such a strict regulation, but it is very difficult to call a
client to come in short notice on their weekend.
